I tend to agree here. But with that being said, I believe there is something to be said for a chameleon's ability to recognize a familiar person who he/she trusts. I have seen this a number of times now with my chams.
To me, the issue with the casque looks more like an old abcess. Could have come from anything! I think you have shown true caring and concern for your animal's well being, and as long as you heed the advice given to you (especially from your vet), i think your cham will recover and do fine.
That's an excellent idea! You could print up the information on a stiff piece of cardstock and then have it heavily laminated so you don't have to worry about it getting wet or soiled. It will just rinse out wipe off.

And as a secondary note, I wouldn't trust the commercial airline not to lose or abuse my baby! I recommend just using ship your reptiles' service, just to be safe. And you can claim their costs on your taxes as a moving expense.
