Can I ask where you got them? Mediterranean Chameleons aren’t that common of a kept species, especially here in the states.
Not a lot of care sheets or captivity knowledge on these guys, but I’d assume it’s care is similar to a veiled chameleon? They live in somewhat the same religion.
T5 is the way to go, they’re known to be more efficient at delivering UVB rays. I would go with linear as they’re more reliable and safe option for getting you chameleon the UVB it needs.

So, give him a few days and not feed him, then see if he’ll accept food, if not. Id give him 2 syringes of repti care a week, then repeat the process of trying to get him to eat. Syringe feeding is really stressful for these guys.

So with feeding, if you’re using the flukers repti care (whatever it’s called) you need to be careful with calcium intake as you can give them too much of it.

I also see the purple bulb, is that a night heat bulb? If so you should just completely get rid of it, they don’t need nighttime heat, they benefit from a nighttime temperatures drop.
He’s either had MDB in the past or currently has it or developing it. He’s got a bent casque, meaning he didn’t get right supplementation or lighting.
