Go to your image and click "image tools" in the upper right-hand corner. Click "edit image." Once there, at the top of the cage, you'll see the following text:

To delete this image, check the box to the left and then click the 'Delete This Image' button below.

Check the box then click the button.

Now, all of this being said, I did not see any image uploaded three times, so either you didn't upload it three times, or a mod took care of it for you.
