Chameleon Info:

Your Chameleon - Male Jackson chameleon, i got him mid September of this year
Handling - maybe once a week now so i dont stress him out
Feeding - I did have hijm on crickets but then i introduced mill worms and he wont eat crickets really. I was feeding the crickets the Flukers food and drink that the pet store recommended and i did put powder on them. but the mill worms i put powder in their little cdontainer once and they said you didnt have to do anything to feed them or anything so i listened and idk if that was smart
Supplements - tetra fauna reptocal
Watering - I have the monsson RS 400 misting 10 seconds every 4 hours to keep humidity up i havent used much hand held sprayers cuz he was drinking off leaves fine
Fecal Description - its brown and white and seems to do it normally
History - I have had a bearded dragon and snakes before in the house so i have some expereience and i researched alot before i got him and had the tank set up but i need help as i have never had a chameleon before

Cage Info:

Cage Type - Glass cage, 12x12x24 approximately its the beginner terrarium
Lighting - exo terra 150 watt bulb and lamp and a 26 watt UVB bulb the one the lady at the pet store told me to get
Temperature - What temp range have you created (cage floor to basking spot)? 60 degrees on bottom and 90 degrees on top and about 70-80 in the middle of thecage. Lowest overnight temp? 60 degrees F How do you measure these temps? I have two thermometers
Humidity - What are your humidity levels?50 usually and then during the night when the lamps are off they go pretty high. How are you creating and maintaining these levels? I have regular mistings and an extra mister if i need too. What do you use to measure humidity? a dual thermometer and humidity gauge
Plants - fake 2 larger ones he drinks off of
Placement - It is on a table so it is higher
Location - USA

Current Problem - He is very skinny now and his eyes are sunken in. I have not been able to get him to eat very much or very often. He is also very weak he has been staying at the bottom and acting like he cant hold his head up so he rests it against things. I got him out to make sure he wasnt dead and his grip is also very weak. I just saw him move and he is stumbling. Im not sure what to do at this point because i know chameleons are more difficult to care for but idk if its something im doing or if he is just sick. Please help! If there are more questions please ask! thanks!

At the top of the health clinic forum page is a sticky titled "how to ask for help". Copy and paste it into a new thread. Complete it in as much detail as you can. It will help the knowledgeable keepers give you the best possible complete advice.

Recommendation

By no means am I an expert, but at this point you likely need a vet visit.
Then you should review the care sheet provided in the care resources at the top left panel, because the diet, supplementation and gut load are not ideal to say the least.

Your chameleon sounds like it is dehydrated. You should increase your misting duration. I have a monsoon too and run it every four hours for 120 seconds. Your supplement is also not correct. You should dust your staple feeders with a plain calcium every day but twice a month you should use a calcium with added D3 and twice a month dust with a multi vitamin. I also think your basking spot is too hot but not 100% sure on that. You should locate and read the care sheet for your cham. They are to the left under care resources. Meal worms are not a nutritious feeder. Think of them as fast-food. Tasty but bad for you. They are very high in fat. The commercial gutload like fluker's is that great either. Many use cricket crack or bug burger. I make my own following the recipe on Sandrachameleon's blog. Pet stores really don't provide proper information on chameleons.

I agree with Ridgebax, Jax preferred method of drinking is to let the water drip on their heads so it can run down into their mouth.
They will lick leaves, but that is more how panthers and veiled get their water. Montanes are different.
They become dehydrated very quickly.
You need to dump lots of water on them, use a dripper system and also a misting bottle.
Water should fall directly on him for several minutes every day.

If the become too dehydrated, they will have organ failure, and when that happens, they will no longer take food or water.
A vet would have to hydrate him via IV or syringe with saline solution.

Get your pet to a vet PDQ, or you may very well loose him :(

150 watt exoterra bulb plus a UVA sounds like you're cooking him IMO. I had a UVA with my megaray bulb recently and I was overheating my Cham. I've recently done away with all of it and got a 10.0 UVB reptizoom and a 50 watt UVA heat repti glo bulb. Her bask temp is exactly 81 degrees mounted 4" above her mesh enclosure from her basking perch. My digital thermometer was off too, my new one is more accurate.

You can't "wing" the temps. They really do need to be spot on. Now my Cham looks better than ever.

All the best lights in the world mean nothing if you're using them wrong. I realized this a few days ago. What good is a $50 dollar megaray bulb if I'm cooking my Cham? I listened to experienced keepers and they helped get me back on track, so make sure to do everything they say.

Also mealworms are not very juicy. They don't hold a lot of water to begin with. I've never seen a mealworms burst in my chams mouth. A hornworm, a well gut loaded cricket, a silkie, a dubia, all of these they get hydration from and iv seen burst with all kinds of juicy crap when my Cham eats them, mealies are like French fries. I give them with a wax worm for a treat .

If he is as bad as you say I'd get him to a vet ASAP to get hydrated by a medical professional. Best wishes.

glad you came here with your concerns - it sounds like you've not had good advice from where you got the chameleon and so your husbandry is not ideal.

I suggest

vet visit
more water (try adding a dripper)
less heat
much better prey selection (mealworms are not a good choice and should form no more than 10-20% of the diet and they DO need to be fed).
learn to gutload properly
learn to supplement properly for your type of chameleon
