I am not a vet but to me it looks like it might be some sort of infection.

I wonder if he might have pulled on a nail to the point of breaking a toe? Just guessing here, but the middle toe doesn't seem to line up with the others.

Looks like damaged (missing/pulled) nails. Can be quite common depending on the type of screen you are using

Does this mean that he will be cut off?
I love my chameleon

Yeah it looks like he may have caught the middle nail on soemthing and it freaked him out and tried to pull and broke something or maybe pulled the nail out. Could also just be an infection behind the nail which the swelling is causing it to push out.

Is that always, or just if it is broken at a certain point?. My chameleon is missing one and I still til this day do not know how. Luckily there was never any blood or type of infection

That happend to my Sebastian aswell. It really hurt for him, I tried to touch it and when ever I did, he would pull away. His climbing was not the same BUT after a normal shed, it fell out naturally. DON'T try to pull it out. So far, his nail has not grown back, but atleast it doesn't hurt anymore.
