A cricket or superworm every few days even when other crickets were in cage did not seem right.I don't think this is on you. Nothing you have done would kill a cham in 3 to 4 months. Even if you found a great vet, there was likely an existing issue when you got him.

Chams do get spoiled with superworms and will refuse other feeders. But I agree with others that there most likely was an underlying health issue that he had before you got him.
