I personally bought my Veiled from FLChams.com and have had no issues so far.
That’s what a lot have told me I’ve done a cleaning of the sticks in the cage but I get like an eerie feeling around the corner where he was dead and flashbacks of his limp little pale body
 
I'm so sorry for your loss, and now that you say the eyes sunk in suddenly, I believe that it was illness. Your husbandry looks good, so don't think that it was your fault, unfortunately, if you keep Reptiles in really good health, you will have to part with them eventually. If you get a chameleon in the future, I would change to a different calcium/multivitamin schedule as Reptivite has preformed Vitamin A, which can be overdosed. Again, I am really sorry for your loss, hope the little guy rests in peace.

With d 3 from a recommendation from my friend who keeps many chameleons. I feed my insects with fresh vegetables such as dandelion, cucumber, and carrots. I do use sometimes a zoomed cricket block.
 
Generally the supplements recommended are...Phosphorous-free calcium at almost every feeding, phosphorous-free calcium/D3 powder twice a month lightly, and a vitamin powder twice a month. Be careful of vitamin d3 and prEformed vitamin A because they can build up in the system and lead to health issues and the two are antagonistic to each other too and need to be in balance.
