If I was to get one like this but for double bulb could I add a heat bulb to it?

NO you can't add heat to it. You must have a heat bulb in a dome fixture and a t5 fixture with a uvb bulb. The only thing you can add to a double bulb T5 fixture besides the uvb is a daylight bulb. This brightens up the cage and is good for live plants.
 
So just so we are clear, here is what you need for lighting, both for UVB and for heat:

UVB Fixture:

Use a regular household incandescent (NOT LED) bulb in it, dimmed so you get the correct BASKING temperature

Ok so I can just order the uvb light and just keep using my 60w blue light?
 
Will this be ok if I just order a new fixture?

I think OP maybe getting confused with the amount of people giving information all at once. Granted it's all good information and accurate. @Grasshopper you need to read carefully and make a list of things to buy and buy them now! Your chameleon is in serious risk of dying. Don't try and take the easy cheaper way out like you have been. There's no "can I get this instead?" or "I can't afford that right now" when it comes to carrying for chameleons. They are unlike any reptile you've ever owned or will ever own again. Please please please fix your husbandry issues ASAP before your chameleon is done for. This is serious!
 
Taking her to the vet Monday morning, he told me until then keep doing the showers and mist more than normal. (No emergency exotic pet near me open now.) Question about the lights, I read for sure they get the 12 hours of both heat and uvb. Also read I should turn the uvb light off 15 minuets after the heat lamp. Is that right?
