Your chameleon also has MBD that has not been treated and that is why his growth is stunted. The parasites can effect that too, as they will not eat but your main concern is the MBD.
 
Correct his supplementation and gutload...overdoses of vitamins like D3 which has already happened in the past can create some of the symptoms of MBD (ironic isn't it?). If he was getting the supplements as you first described I think this is a lot of the problem. Also get him out into some real sunlight. The limb defects won't really go away but you can keep them from getting worse in future. He will probably always look a bit off and be small, but he can still have a decent life from now on.
 
The prolapse should not have anything to do with him not using his tongue. if this suddenly just happened, then he might have hurt it somehow. You are saying, he just stopped using it??

I was thinking it was the MBD, but I didn't think about if it just happened suddenly. If that is the case, he could have hurt it. Usually with MBD it starts out as them just missing the food, then it wont shoot out as far, and eventually they cant use it at all. Or, if he could just not want to eat anything right now, but if he is willingly taking food from your hand, then I would think that is not the case.
