Start by reading through the care sheet and trying to get you're husbandry in line with what it says.
I will say a few things
1. A night heat source is not needed unless the temp drops below 55 at night. If it does you can use a ceramic heat bulb that puts off no light. Chams need complete darkness to sleep.
2. Suppliments. The schedule
Most keepers follow is...
Calcium with NO D3 daily
Calcium with D3 twice a month
Multivitamin 2x a month
Check your multivitamin, if it has D3 in it then you do not need the additional calcium with D3.
D3 can build up and cause toxicity, so you only want to use it at the most twice a month.
3. The UVB light should be on for at least 10-12 hours a day. They need the UVB and heat lamp during the day. Your basking temp needs to be a bit higher. More like 85. You can just get a higher watt house bulb to raise it. I would suggest getting a linear UVB bulb that goes in a hood rather than a screw in light bulb type. It will cover more area.
I also agree you need to take her to a vet and take a fresh poop sample with you. That way the vet can check for parasites.
