Maybe dehydration? Maybe MBD?

Supplements and feeding/gutloaded the insects to prevent MBD and other health issues....
Since many of the feeder insects have a poor ratio of calcium to phosphorus it's recommended that you dust the insects before feeding them to the chameleon with a phosphorus-free calcium powder at almost every feeding to help make up for it.

It's recommended that you dust twice a month with a phosphorus-free calcium/D3 powder to ensure the chameleon gets some and leaving it to produce the rest from it's exposure to the UVB light. D3 supplements can build up in the system and lead to health issues but D3 from exposure to the UVB won't as long as the chameleon can move in and out of it at will.

It's also recommended that you dust twice a month with a vitamin powder that contains a beta carotene prOformed source of vitamin A (no D3). PrOformed sources won't build up in the system like prEformed sources will....but this leaves it up to you to decide if/when the chameleon needs some prEformed vitamin A.

Crickets, roaches, locusts, superworms, can be fed/gutloaded with greens, veggies and a bit of fruit such as dandelion greens, kale, endive, escarole, collards, carrots, zucchini, squash, sweet red pepper, sweet potato, berries, apples, pears, melon, etc.

I don't see nay signs of MBD but with the history the above information should help you prevent the problems.

I'm still concerned about the nose in the air and the swollen eyes.
 
Thank you all. We have the dusting schedule all laid out and will be vigilant in making sure his diet is well rounded.
His light is at the top of the vivarium, on one side so that is able to get to different areas of heat/light.
His area is on a wall with no window but has a window on the left that is approx 8 feet away. The sunlight does not directly shine on the vivarium. There is a vent on the ceiling but is approx 6 feet away and does not blow into his vivarium.

This morning... he is in the same spot as last night and no longer has his nose in the air. His eyes are still closed.
