How big are the insects you're feeding the chameleon? If you want to know the sex look at the heels...if there is a little bump/flap of skin on the heels it's male...if not it's female....

Perhaps the blue daylight bulb is part of the problem...try replacing it with a regular household incandescent bulb of a wattage that provides a basking temperature in the low 80'sF and see if it makes a difference.
 
Definitely read through the caresheet in the resource tab here on the forum. Make sure your cage and supplies match everything in the caresheet and such. Reading through your help form I would say fill the cage with more various sized vines and a lot more foliage to reduce stress for the next chameleon. Also, get the proper supplements and a good schedule for supplementing.

Cal no D3 every feeding, Cal w/ D3 twice per month (one day every other week) and a multivitamin twice per month alternating with the D3.

I bought my male Veiled from FLChams and I have no complaints. The 4 month old chameleon arrived healthy and settled in the his new home nicely. Started out eating 10-12 small crickets every day and now, after alittle more than a month in my care, he is eating 12-16 small crickets every day.
