Your Chameleon - The species, sex, and age of your chameleon. How long has it been in your care? She's a female Jackson's chameleon and has been in my care for about a month.
Handling - How often do you handle your chameleon? Maybe once a week.
Feeding - What are you feeding your cham? What amount? What is the schedule? How are you gut-loading your feeders? Crickets mostly, was feeding a superworm as a snack every other day. I give her a dusted cricket w/o d3 once a week, and vitamins and cal w/d3 once a month. Crickets are gutloaded with carrots, oatmeal, apples. I feed her 20 mins after her lights come on.
Supplements - What brand and type of calcium and vitamin products are you dusting your feeders with and what is the schedule? ^
Watering - What kind of watering technique do you use? How often and how long to you mist? Do you see your chameleon drinking? I mist her 3-5 times a day and she has a dripper that drips 2 drops every 10 seconds.
Fecal Description - Briefly note colors and consistency from recent droppings. Has this chameleon ever been tested for parasites? Since Ive had her she's had good brown/black poo but white/orange with clear slimey stuff? Haven't got her tested yet or had a fecal done.
History - Any previous information about your cham that might be useful to others when trying to help you. Got her from Petsmart (yeah, yeah I know...)
Cage Info:
Cage Type - Describe your cage (Glass, Screen, Combo?) What are the dimensions? 18x18x24 Exo terra bottom with a 17x17x20 screen addition to the top. So 18x18x44
Lighting - What brand, model, and types of lighting are you using? What is your daily lighting schedule? Repti glo 5.0 uvb bulb, and a 40w florecent bulb for basking spot. 12 on 12 off 8am - 8pm
Temperature - What temp range have you created (cage floor to basking spot)? Lowest overnight temp? How do you measure these temps? 80 up top by the basking spot and 72ish on the bottom. 72 is what my apt ac is set to so that's about as cold as it gets.
Humidity - What are your humidity levels? How are you creating and maintaining these levels? What do you use to measure humidity? Stays no lower than 50%. I mist 3-5 times a day and have 2 live plants and soil covered with screen and then flat rocks on top of it to keep humidity and stop my drainage problem.N
Plants - Are you using live plants? If so, what kind? Umbrella plant and pothos plant.
Placement - Where is your cage located? Is it near any fans, air vents, or high traffic areas? At what height is the top of the cage relative to your room floor? It's in my dining room where we don't eat, tucked I the corner. I do have a dog and cat but they usually don't play in that area.

Current Problem - The current problem you are concerned about. Have noticed she's stopped hunting lately, well in front of me atleast, she used to eat in front of me. I've seen her drink from the dripper off a leaf once in front of me. She's got a slimey unriante, and has been closing her eyes later during the day. Usually 1hr or so before lights out. Help?! Please!?
