Need help cham sick bad!

well got home today walked in my room found my 2 year old veiled named sage laying on the ground black and yellow, but not sitting on the ground but like on his side. Immeditly i noticed his eyes were sunken which ive never ever seen before which this morning he was happy basking in the light. grabbed the light attached it to the side of the cage and held him a foot under it. He couldnt stand but barely do anything. I gave him water slowly. I would of took him to the vet but the closest is 3-4 hours away. we kept giving him water and then he stuck out his tongue and held it there. you could see him breathing and his pulse but now its gone :( We actually just moved his cage across the room (4 or 5 ft) because im getting a new bed and was happy last night when i went to bed. Im prety shure he passed away now but his skin keeps changing. anything? plz help!

Cage Type - Screen 24 24 50
Lighting - 80 watt heat lamp then housse hold 40 and a uvb light i think? Exo terra REpto glow 5.0 UVB
Temperature - 95F is the basking temp then at night mybe 68F Electronic meter
Humidity - 65% electronic meter
Plants - just took them out this summer because were dieing, maybe in july?
Placement - by a alrge window looking out a beutiful veiw down a hill
Location - northwest indiana

Chameleon Info:
Your Chameleon - Veiled chameleon male, 2 1/2
Handling - once a day
Feeding - crickets and ocasionally mealworms
Supplements - T-rex cameleon supplement, gave to him every other feeding
Watering - HAve a clean large foutain tht he loves also i mist alot each day with squirt bottle
Fecal Description - always bright yellow and green
History - nope
Current Problem - above
 
Well knowing the supplements would really help in this case. Any way you can tell us?

that link may help u find a closer vet. good luck, dude. you'd need 3 types of supplements for a veiled chameleon kept indoors: calcium powder with d3 (1-2 x monthly depending on brand and trips outside, etc), calcium powder without d3 (used every to every other feeding), and herpevite or eq. multivitamin once a month. supplementation can help in the long term, but in the short term it looks like your cham needs a vet pronto, tho i'm no expert.

regarding the fountain, are you sure he doesn't poop in it ever? these types of things are not recommended for chameleons.
 
umm only crickets he can mostly eat. there are a few fake plants but u cant bite those ones and theres no bite marks

Sounds like you didn't have a UVB bulb and we need to know the supplements...but its possible it was a calcium imbalance.

Do you have any pictures of him?
 
ok i updated the supplements and the light stuff. My only picture of him is in my avatar but he was really healthy and happy
