Do you feed it worms?
I think it is having difficulty passing an undigested worm or other food or substrate and it got stuck.
If it is not that, then worse case scenario it is a prolapse, either way a visit to the vet is probably yours and his best bet.
Take this advice if no one else responds in 24 hours,

Hopefully... although a week is a long time for a chameleon to have suffered a prolapse without being seen. As Camimom has said you need to get to a vet URGENTLY - your chameleon is probably in a deal of discomfort and needs to be treated as soon as possible.

since its probably too dry to go back in yes.
they can do surgery to remove the hemepene (assuming that's the part it is).
if its not too dry, they can surgically put it back in, put a stitch in the vent, and it can heal..
however, at this point, removal is probably the best step. esp if its been out this long.

Can you tell me what part of the world you are from if you need assistance finding a rep vet please?
I'm in Alexandria VA. I called one reptile vet and they're saying to fix it surgically could be $500 - $1000. Of course I want to get the problem fixed but I'm not able to spend that much money.
When I first noticed it, I didn't know what it was but it didn't look that abnormal. I thought maybe some fecal matter was stuck and would come out on its own. Now I feel stupid for letting it go so long.

Call best friends animal hospital in Gaithersburg md.
Dr Hoffman is fantastic. I take all my chammies to her. she just did a surgery for me to remove one o fmy chameleons tongues.
shes in surgery today, but you can try for an apt for tomorrow.
I actually have my own apt scheduled tomorrow, if they are full, PM me, and you can take my slot.

It is not going to be cheap, but you have the cheaper option, which is euthanasia which would be preferable to the slow agonizing death that will follow if left.
Sorry.
Most vets are happy to enter into a repayment system, I can only urge people reading this to bare in mind they need to keep a good emergency fund for these creatures as with other pets.

You need a vet immediately. Like previous comments suggest...it is prolapse. I dont know what the vet will be able to do. From the pic it seems like it has been so long it may not be able to be treated the way it could have been. Keep us updated

the things I suggested are what the vet will do. either remove it, or get it back inside. those are the options. the vet can determine which option is better though.
im leaning towards amputation though.
and if that happens, its nto soo bad, he can live without it. as long as there isn't other damage, no infection, and he isn't super sick from the week its gone on.
