Dont try to hand feed. Put the food in the cage (or in feeding cup suspended in an appropriate place) and walk away. He's probably too shy to eat in from of you. Using a feeding cup for the crickets will help you see how much he is eating.
I essentially do what Sandra does. Calcium almost every feeding, D3 a couple times a month, vitamin supplement once or twice a month. Important to gutload as well.
These are okay for when you're in a hurry. But if you can make your own gutloads, especially fresh fruits and veggies, I think the chameleon will be better off in the long run.

Why not? Irradiation doesnt effect their nutritional value, just ensures they dont breed. A lot of the fruit we get here imported from Mexico and south america is irradiated to kill off bugs. It doesnt hurt anything. The effect is temporary.

Mine has slept hanging upside down from the top of the cage, on the sides facing upwards or downwards, and every angle in between. Apparently its normal. They seem to have a good grip (toenails help) even when asleep.
I think you're already offering a good selection with roaches, silkworms, hornworms and crickets, with a few mealworms and waxies. If you're worried bout supers, dont add them. Your chameleon is likely getting what it needs through the choices you already offer, especially if you gutload well...
