HELP with veiled chameleon!

Chameleon Info:

Your Chameleon - The species, sex, and age of your chameleon. How long has it been in your care? vieled male, not sure of age, approx 3-4" long
Handling - How often do you handle your chameleon? trying not too. maybe 4-5 times since i purchased him
Feeding - What are you feeding your cham? What amount? What is the schedule? How are you gut-loading your feeders? pinhead-1/4" crix 10 pinheads in feeding bin, 6 1/4" freeranging in cage, fluckers high calcium gut load with blue fluckers water crystals. fresh cirus friuts and tomatoes, lettuce,
Supplements - What brand and type of calcium and vitamin products are you dusting your feeders with and what is the schedule?Reptivite (with and without D3) havent used the "with D3" yet. lightly dusting "w/o D3" daily.
Watering - What kind of watering technique do you use? How often and how long to you mist? Do you see your chameleon drinking? hand misting 5-6 times a day for the entire cage. I also have a little dripper filled twice a day slowy dripping on top of schefferela plant.
Fecal Description - Briefly note colors and consistency from recent droppings. Has this chameleon ever been tested for parasites? not tested for parasites yet, droppings colors are brwon with white tip after first day home to orange/yellow with white tip yesterday.
History - Any previous information about your cham that might be useful to others when trying to help you. nothing to report from local store.

Cage Info:

Cage Type - Describe your cage (Glass, Screen, Combo?) What are the dimensions?18x18x36" reptibreeze
Lighting - What brand, model, and types of lighting are you using? What is your daily lighting schedule? 5.5" basking lamp with 75 watt house bulb, 18" reptiglow 5.0 linear uvb
Temperature - What temp range have you created (cage floor to basking spot)? Lowest overnight temp? How do you measure these temps? I purchased a dual analog thermometer/hyudrometer from store but it must be broken cause it doesnt work and store is closed till wendsay due to memorial day. last few nights have been muggy temps in high low to mid 60's upper 80's low 90's during the day.
Humidity - What are your humidity levels? How are you creating and maintaining these levels? What do you use to measure humidity? see above
Plants - Are you using live plants? If so, what kind? 30" schefflera arboricola, large plastic ficus plant
Placement - Where is your cage located? Is it near any fans, air vents, or high traffic areas? At what height is the top of the cage relative to your room floor? in my bedroom next to my bed on small 28" tall dresser 6 ft away from window and small circular fan in room. no a/c in apt.
Location - Where are you geographically located? Suffolk county NY

So I took her to the vet this morning as soon as they opened. They gave her something to stimulate her appetite, a deworming med and an antibiotic. They force fed her a little bit too. She seemed to be doing much better when I brought her home but about an hour and a half ago she passed away. I guess unfortunately it was just too late for her. Although I'm glad she's no longer suffering, I am very sad it turned out this way. I wish I could have gotten her to the vet before the long weekend and maybe things would have turned out differently. The emergency vet in our area doesn't have a vet that is able to care for reptiles. It's just extremely unfortunate and I feel so bad that she suffered so long. At least I know that I did everything I could for her up until the last minute. Words of advice: if you even suspect that something is wrong, take them to the vet, don't wait to confirm your suspitions because it may be too late. :(

The first commandment for beginners at cham keeping. I suspect most people learn it the hard way.
